Caregiver Connection Center

Caregiver Connection Center connects informal caregivers in the 46619 zip code, one of the neediest neighborhoods in St. Joseph county, to a comprehensive network of resources to assist them as they care for our community's older adults.  This is accomplished through case management, caregiver education, and direct services.  A trained geriatric case manager provides caregivers with unfettered access to information, resources and services.  Caregivers participate in a series of training sessions that are tailored to address the specific needs of caregivers and are taught by a BSN employed by Memorial Home Health Care and Alzheimer's Services staff.  Caregivers who participate in training may receive direct services that include home modifications, transportation, and respite care.
