The goal of Case Management is to help keep people living in their own home as long as possible, delaying or avoiding nursing home or group home placement. Case Management is a service provided by trained professionals who assess needs and coordinate services. Case Managers meet with the person in their own home and conduct a confidential, thorough assessment of their health & safety needs. Case Managers will ask people about their diagnosis, their medications, perform a nutrition screen, attain financial information to determine eligibility for benefits, and ask and observe how the person is managing their daily activities in their own home.
For all people, but especially with people with Alzheimer’s or other dementia-related ailments, people who have a guardian, or people with communication difficulties, family or other caregivers are encouraged to be present at the assessment. An example might be the person might claim they can bath independently, but actually they frequently forget, or cannot bathe adequately to maintain appropriate hygiene. The family or caregiver can provide accurate information to the Case Manager, so that the Case Manager gets accurate information.
Through this assessment, the case manager can determine the strengths, weaknesses and specific needs of that individual. The Case Manager can then pass along information on programs or services that may be available to the person to help keep them safely in their own homes.
Through questions and observation, a Case Manager can make a determination of appropriate living situation (Is it safe for mom to be living alone at home? Does she need or qualify for home care services, and who or what programs can pay for the needed care? Would mom be safer in an Assisted Living program and where are those facilities? Is mom qualified for nursing home placement and is that placement necessary at this time?)
There are no age or income requirements to request an assessment. Case Managers are well versed on serving the elderly, as well as serving those who are younger, but may have other physical impairments, such as people with Multiple Sclerosis, Muscular Dystrophy. Case Managers also specialize in serving people with Mental Retardation/Developmental Disabilities.

Case Managers can develop an individualized Care Plan, which details the type of services needed, the frequency, and the payor source for each service. Medicare, Medicaid, private insurance, other family or friends who provide support, and public services or other programs used to help keep that person at home are included in the planning stages while developing the Plan of Care.
An initial assessment is conducted free of charge. On-going Case Management can be funded by an in-home services program or by private pay.
