Targeted Case Management

Targeted Case Management is a funding source which is an entitlement program. This program only pays for Case Management services. To be eligible for this funding, people must have a diagnosis of a Developmental Disability and also have Medicaid.

There are two distinct parts to this program: Targeted Case Management Intake and Targeted Case Management On-going.

Targeted Case Management Intake is the starting point. Persons wanting to apply for services can contact their Bureau of Developmental Disabilities Field Service office.

A determination will be made if the individual meets the guidelines in the State of Indiana to be determined Developmentally Disabled. This determination is different than meeting a ‘level of care’ to be appropriate for a DD Medicaid Waiver: DD Medicaid Waiver (formerly called the ICF/MR Medicaid Waiver), Autism Medicaid Waiver or Support Services Medicaid Waiver. Persons who have been denied services from one of Indiana’s Medicaid Waiver programs are encouraged to apply for this funding source.

Once the person is determined to have a diagnosis of a developmentally disability, they have their choice of Case Management entities. Case Managers in Indiana can work for not-for-profit agencies, like an Area Agency on Aging. Case Managers can also work for themselves, or work for a for-profit, private Case Management agency. Individuals can change their choice of Case Manager at any time.

Once the individual has chosen their provider for Case Management, that person can begin providing Case Management services under the funding source of Targeted Case Management on-going (TCM-O). One of the components of TCM-O is completing person centered planning. Person centered planning maps out the dreams and hopes and wishes of the individual. This case manager will also work with the individual to apply for any services or programs, including Medicaid Waiver funding or State line item funding, which the individual may be able to access.
