  • Community Living Specialist: Michigan City/Elkhart/South Bend/Mishawaka 

This full-time position is responsible for providing information, referrals, and options to individuals when they access the Aging and Disability Resource Center and is responsible for addressing client needs and managing the coordination of clients’ on-going care, with a focus on serving clients applying for the Medicaid Waiver for Assisted Living services and clients residing in Assisted Living communities with services funded by the Medicaid Waiver.  This position’s primary functions include initiating Assisted Living services under the A & D and TBI Medicaid Waivers and performing on-going care management functions for Waiver Clients residing in Assisted Living communities.  This position supports the organization’s mission to promote independence and empower individuals served.   • Part-time County Coordinator: Kos County

This part-time position is responsible for assisting in establishing and supervising Nutrition Sites, the training of Site Directors and volunteers, auditing paperwork, performing site inspections, assisting with finding suitable rent-free facilities for the site locations, providing community outreach, programming for
the various sites based on client and program needs, covering for site director absence, as needed, due to vacation, illness or lack of staff, along with other duties as they are assigned. Incumbent will monitor County Nutrition Sites. For the full job description, please click here

  • 2 Options Counselor positions:

South Bend/Elkhart

This full-time position is responsible for providing information and options to individuals when they access the ADRC (Aging and Disability Resource Center). This  position provides phone options counseling , face-to-face options counseling, service plan development and documentation, and assists with application for Medicaid.   • Part-time Meals on Wheels Assistant:

This Part-time position is responsible for supporting Elkhart Meals on Wheels Volunteers.  Assisting with client follow up, start up, routing and getting client data information to the Kitchen Assistants. Assistant is also responsible for coordinating volunteers for meal routes, training new volunteers, and tracking all volunteer hours, collecting all route sheets from volunteers and when needed entering information in the data base.  Position also delivers meals for routes with a volunteer vacancy. For the full job description, please click here

  • Family Development Consultant: 

This full-time position determines enrollment eligibility of a client referral, assists the client in the development of a service plan to enable them to become financially self-sufficient providing mentoring/support, referrals, and follow-up to meet the objectives of the client’s service plan.   •   Part-time A.M Transportation Driver position:

This part-time position is responsible for providing transportation for older adults (who have no other means) to necessary appointments or for errand purposes. For the full job description, please click here

  • 2 Options Counseling Case Assistant:

This full-time position is responsible for support to the ADRC and Options Counseling program and will include maintaining data bases, spreadsheets, and reports that assist in tracking care plans, Medicaid and vendors. For the full job description, please click here
