Open Positions

  • On Call Nutrition Driver:
This on call part-time hourly position consists of 4-5 hours per day when needed, is responsible for delivering food and supplies to the Nutrition Sites over a specified route(s).   • Family Development Consultant: 
This full-time position determines enrollment eligibility of a client referral, assists the client in the development of a service plan to enable them to become financially self-sufficient providing mentoring/support, referrals, and follow-up to meet the objectives of the client’s service plan.   • Billing Specialist:
This full-time position is responsible for assisting with various types of billing including Medicaid Waiver, Waiver Intake, PAS, Choice, SSBG, Title III, Veterans, Private Pay, and others as needed. This position manages the Money Follows the Person (MFP) Sub-Hub billing and reporting. This position also manages Cost Share payments. For the full job description, please click here.

 

  • ADRC Options Counselor:
 This full-time position is responsible for providing information and options to individuals when they access the ADRC (Aging and Disability Resource Center). This position provides phone options counseling, face-to-face options counseling, service plan development and documentation, and assists with application for Medicaid. This position will provide decision support to inquirers while assisting them in making educated decisions about options that are available in their community to maintain independence.   • Director of Alzheimer’s & Dementia Services of Northern Indiana:
This full-time position is responsible for the planning, development, implementation and monitoring of Alzheimer’s and Dementia Services of Northern Indiana, a division of REAL Services. For the full job description, please click here.

 

  • Housing Specialist:
This part-time position is responsible for the management of intake through completion, for all home modification programs. Administration of non-weatherization housing programs will be the responsibility of this position. For the full job description, please click here.
